About The Role

We are currently seeking a dementia services co-ordinator to join the Age Exchange team. The role is based from the Blackheath office, and will require work in both Blackleath and Lambeth, plus approximately 4 trips to Manchester per year. At the moment, the role is home based with the occasional day on site.

Age Exchange has been improving the health and wellbeing of older people for over 30 years, we specialise in the practice of Reminiscence and in bringing creativity into people’s lives. Following a partnership with care provider Community Integrated Care, we have started to expand and want to take our specialist Reminiscence Arts intervention for people with dementia to other parts of the UK. In March 2019 we opened a service in Manchester and in August 2019 won a contract to provide 3 weekly services in London Borough of Lambeth. It’s a really exciting time for us and a chance to achieve our vision of increasing the number of people who benefit from our work.

We are looking for someone to help us reach our goal of expansion alongside ensuring that the quality of the service we provide is maintained. The post holder will be responsible all of our dementia and carers services that are outside of our Blackheath base. They will support the development of relationships with commissioners, voluntary sector agencies and local influencers where we are planning to set up new services and then overseeing those services. Specifically you will be responsible for delivering the new services in Lambeth and Manchester. You will lead on developing referral routes, assessing referrals, managing the reminiscence artists and coordinators who deliver the services and ensuring a high standard of delivery and safeguarding.

You need to be committed to improving the lives of people with dementia and those who care for them with a belief that creativity can have a positive impact on health and wellbeing.

This is a full time role for one year to cover maternity leave.

Interviews will take place via video call.

Essential criteria

About You

We are currently seeking candidates who have experience of delivering services for people living with dementia and their carers, as well as the ability to assess and evaluate client needs. The role requires experience in developing and managing stakeholder relationships and strong networking skills.

Knowledge of relevant legislation such as the mental capacity act and health and safety would be beneficial.

Excellent communication skills and a high standard of computer literacy are essential to the role.
